All screens Nationwide Ltd is seeking a compassionate Rough Sleeping Case Officer to join the South and East Lincolnshire Councils Partnership. You will deliver person-centred support to people sleeping rough or at risk, helping them access accommodation and sustainable housing.
The role requires street outreach, building trusted relationships, coordinating with housing providers, health professionals and support agencies, and contributing to multi-agency plans.
