• Lead clinical case reviews and ensure high standards of patient care

• Mentor and provide oversight to advanced practice clinicians across multiple facilities

• Collaborate with pharmacists, nurses, and leadership on medication management, care coordination, and quality initiatives

• Build strong community relationships and represent the practice with professionalism

• Develop and implement individualized patient care plans

• Provide timely charting and response to pharmacy recommendations

• Board-certified physician (FM, IM, EM, or related discipline)

• Background in post-acute, skilled nursing, home health, hospice, palliative, or hospital medicine preferred

• Prior medical director experience is a plus but not required

• Fee-for-service structure; annual earnings typically range from $310,000 $ 345,000+ with uncapped productivity potential

• $25,000 sign-on bonus
