Social Worker

Schedule: Full time, This is a salaried, exempt position.

Duties/Responsibilities:

• Ability to provide supportive counseling and education to residents and families as needed.

• Actively participate in resident care conferences and complete documentation in accordance with facility guidelines.

• Knowledge of long term care rules and regulations required.

• Deals well with a diverse client population ranging from the geriatric population to younger rehab residents with varying physical/social issues.

• Works well in a busy environment with multiple competing priorities.

• Discharge Planning

Requirements/Qualifications:

• Two years experience as a Social Worker dealing with the adult/geriatric population, preferably in a long term care setting.

• Degree in Social Work or closely related field beneficial.
