RN Liaison
- Hospice-Weekend Track
- Eligible for Full-Time Medical Benefits. The Community Hospice has a rewarding opportunity for a RN to work as part of our Hospice Intake RN Team serving as a Liaison/admission RN/hospice RN seeing patients in local hospitals (Capital Region—primarily Albany Medical Center, St. Peter's Hospital).
Only every Saturday and Sunday 7 am—7:30 pm. No days during the week after training period.

- Evaluate patients for eligibility and interest in hospice services, describe the support and care available from hospice along with the benefits of hospice care.
- Coordinate with the hospital providers and our hospice medical directors to confirm eligibility for hospice services.
- Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.
- Admit patients to hospice inpatient services upon eligibility.
- Develop a hospice plan of care and collaborate with the hospice interdisciplinary team and the acute care team.
- Work closely with the hospital case management department.
- A current license to practice as a Registered Nurse in the State of New York
- Associate’s Degree in Nursing, BSN preferred
- Minimum 1 year of recent acute care RN experience
- Hospice or RN Liaison experience a plus but not required.
- The RN must be able to communicate effectively, verbally and written.
- Valid Driver's License and reliable vehicle.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
- Understanding of Hospice principles preferred.
Training provided in end‑of‑life care.
Community Hospice provides a thorough orientation, preceptors and a supportive work environment.
Enhanced rate for weekend track: $57.90/hr
