Department/Unit: Inpatient Rehab – E8 Work Shift: Day (United States of America) Salary Range: $50,639.00 – $75,959.00
The Licensed Practical Nurse (LPN) provides direct patient care under the direction and supervision of a provider, registered nurse, or clinical care coordinator as prescribed by the New York State Practice Act. The LPN delivers patient information to the health‑care team that influences planning, provision, and evaluation of patient care. The LPN administers medications and vaccinations, observes, measures, records, and reports clinical data related to a patient's health status, performs clinical procedures such as urinary catheterization, tracheal suctioning, and sterile dressing changes, and offers emotional support and educational material to patients and families.
LPNs supervise care provided by unlicensed assistive personnel, including medical assistants, patient‑care associates, and patient‑care technicians; may start an IV and administer blood products under recommendation of their manager and additional specialized training; and are responsible for reviewing medication therapy plans. No LPN, including LPNIII, may assess central lines or titrate medications.
Core Responsibilities- Administer medications and vaccinations as per scope of practice and competency documentation
- Observe, measure, and record clinical data, and report findings to the health‑care team
- Perform clinical procedures such as urinary catheterization, tracheal suctioning, and sterile dressing changes with verified training and competency
- Provide compassionate, individualized care and emotional support to patients and families
- Supervise unlicensed assistive personnel and ensure safe, competent patient care
- Assist in coordination and implementation of individualized plans of care
- Adhere to the NYS Nurse Practice Act and hospital policies, including safety protocols
- Graduated from a licensed practical nurse academic nursing program (vocational school diploma)
- State licensure as a Licensed Practical Nurse in New York upon hire
- Applicable experience per department hiring guidelines
- Demonstrated ability to improve job performance through continuing education
- Proficiency in quality concepts, problem‑solving tools, and patient‑care quality improvement methods
- Effective communication skills with patients, families, and staff
- Read comprehension of patient records and safety procedures
- Mathematical competency – addition, subtraction, multiplication, division, and interpretation of rates, ratios, and percentages
- Critical‑thinking abilities to identify problems, gather data, establish facts and draw valid conclusions
- Standing, walking, and sitting for prolonged periods
- Frequent lifting, carrying, pushing, pulling, and occasional climbing, balancing, stooping, kneeling, crouching, crawling, reaching, handling, and grasping
- Repetitive motions, eye/hand/foot coordination, and constant talking and hearing
- Exposure to extreme cold, low heat, humidity, wet conditions, noise, and hazards; frequent temperature changes
