Position Overview

Under the direction of the Charge RN, the Licensed Practical Nurse (LPN) will participate as a member of the Residential Treatment Team to ensure an optimal level of medical care for the children herine’s Center for Children. The LPN helps manage the activities of the health office and participates in the day to day operation of the residential program.

Organizational Relationships
  • This position reports directly to the Director of Nursing. The LPN acts as a liaison between internal and external medical service consultants, and Residential Program staff.
  • The LPN participates in orientation and professional training pertinent to position and is familiar and complies with guidelines, policies, and procedures as outlined in the agency manual.
  • The LPN represents the medical department at pertinent treatment reviews, special meetings, or assigned agency committees, and participates in regular meetings with supervisor.
Requirements
  • Current Licensed Practical Nurse Credential (LPN), registered to practice nursing in New York State,
    required
  • Associate's Degree and at least 1 year of nursing experience required
  • Current CPR/First Aid Certifications
  • Local travel is required; a Clean and Valid New York State Driver’s License (actively licensed at least one year)
    - required
  • Proficiency in various computer applications:
    Microsoft Office and Google Business Workspace Suite, and use secure Internet practices
  • Experience with AWARDS / NYS Connections, a plus!
  • Capacity and openness to work with individuals from culturally diverse backgrounds; various mental health issues, substance abuse and developmental disabilities
  • Good organizational and time management skills
  • Self-motivated, engaged and highly organized with a strong attention to detail
  • Outstanding communication and interpersonal skills; and exhibit empathy for those in need
  • Demonstrated placidity and patience, flexibility and adaptability.

* Staff in driving positions must be able to meet the agency’s minimum requirements of our insurance policy for authorized drivers including, but not limited to, the minimum age of 21 at time of hire; with the ability to maintain insurability throughout employment.

Essential Duties & Responsibilities
  • Treatment of ill children, including referral to emergency room and all other medical consultants. This would include implementation of all physician’s orders.
  • Make daily rounds on assigned caseload to include: review of medical records in each residential house, inventory of medications, and communication with Child Care staff regarding clients’ care.
  • Maintain complete medical records on all children within the Agency’s care.
  • Assist with weekly pediatric clinics, including implementation of medical orders.
  • Conduct medication reviews, including medication changes as prescribed.
  • Responsible for ordering medications, correct transcription of the order onto the MAR, and communication of medication orders to Child Care staff.
  • Distribution of home visit medications.
  • Attend treatment reviews of assigned clients.
  • Administer immunizations, as needed, to clients.
  • Participate in weekly House Management meetings, staff meetings, team meetings, service plan reviews, and in‑service trainings.
