Capital Cardiology Associates is seeking a full-time Cardio Metabolic Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our clinic.
In this specialized role, the APP will provide comprehensive cardiometabolic care, including cardiovascular, endocrine, weight management, kidney disease, and lifestyle medicine components. The NP/PA will assess patients and develop treatment plans in collaboration with our multidisciplinary team of pharmacists, dietitians, exercise trainers, nurses, and health coaches.
This role requires a strong clinical foundation, understanding of cardiovascular and metabolic management aspects and the ability to deliver patient-centered care and treatment plans.
Including:
- CV risk, metabolic/endocrine, diabetes, kidney disease, sleep apnea and obesity screening and assessment, lifestyle medicine
- Guidelines in obesity management
- Assessing patients’ readiness for change
- Engaging patients in shared decision-making and developing treatment plans that include considerations such as lifestyle and nutrition changes and medication
- Emerging treatment options- including cardiovascular, endocrine and anti-obesity medications
- Patient counseling and motivational interviewing techniques
Responsibilities to include but not limited:
Patient Care Management:- Conduct thorough assessments of patients to assess for cardiovascular, kidney, metabolic risks- evaluating medical history, performing physical exams, and ordering appropriate diagnostic tests.
- Orders and interprets diagnostic and therapeutic tests relative to patient’s needs.
- Prescribes appropriate pharmacological and non-pharmacological treatment modalities.
- Collaborate with multidisciplinary team and incorporate aspects of lifestyle medicine to achieve overall optimal patient outcomes.
- Develop personalized, evidence-based management plans for patients considering their medical history, current health status, and individual goals.
- Monitor patients' progress, assess and manage any complications, adjust treatment plans as needed, and provide ongoing support.
- Monitor the effectiveness of interventions.
- Seeks consultation from supervising physician, as applicable, when appropriate.
- Educate patients about different pharmacological cardiometabolic options- including anti-obesity medications, risks and benefits, lifestyle changes.
- Utilize “down-time” to assist other clinical areas as able (tasks, insurance appeal clinical justification).
- Demonstrate understanding of documentation requirements for level of visit performed.
- Clinical notes are clear, organized, up-to-date and accurate.
- Work closely with cardiologists, pharmacists, dietitians, medication access specialists and other healthcare professionals to create a multidisciplinary approach to patient care.
- Effectively communicate clinical findings and plan of care to physicians, patients, families and other members of health care team.
- Promotes a positive public image for the profession and for CCA.
- Respects confidentiality and privacy.
- Functions as an integral part of the CCA team and participates in the maintenance of a cohesive work environment.
- Assists with training/education of staff, contributes to an open learning environment.
- Functions independently within scope of practice delineated by New York State.
- Maintains required certifications and licensures.
- Demonstrates flexibility in work location to accommodate needs of CCA as a whole.
- Contribute to quality improvement initiatives by collecting and analyzing data, participating in case reviews, and suggesting process enhancements.
- Stay current with advancements in cardio-kidney-metabolic and obesity guidelines, and research to offer evidence-based care.

- Education:
Master’s degree from an accredited Nurse Practitioner or Physician Assistant program. - Licensure/Certification:
