Location: City of Albany
The Community Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Care team serving Albany and Rensselaer Counties.
Schedule:
Monday–Friday 8a–4p, full-time, day shift, no weekends, no on-call, no mandatory overtime.
Provide skilled nursing in patients' homes, monitor status, educate families, support hospice plan, and collaborate with inter-disciplinary team. Valid NY LPN license, 1 year experience, homecare/hospice preferred, driver's license required.
