Community Hospice is seeking a Full-Time Registered Nurse Liaison to join the Hospice Intake Team in Albany, NY. The RN will evaluate hospitalized patients for hospice eligibility and educate patients and families about hospice services, ensuring a smooth transition to hospice care when appropriate.
The role requires NY RN licensure, an ADN (BSN preferred), and 1 year of acute care experience. Travel with a reliable vehicle and a valid driver’s license are required.
