The Registered Professional Care Management Specialist (C3) Team Lead assumes the responsibility and accountability for collaboratively managing the progression of patient’s care with the physician and health care team utilizing evidence based best practice guidelines. The Care Management Specialist Team Lead engages the patient and care team in the joint management of key patient outcomes such as management of disease progression, continued LOS and transition to most appropriate post- acute setting.

The Care Management Specialist Team Lead is a team resource who is focused on the" whole patient experience" not just the acute care episode. Ensures optimal communication, appropriate use of clinical resources and management of patient flow across the continuum. The C3 Team Lead serves as a resource and expert to the staff and provides on going education to staff, patients, and families.

This position requires excellent interpersonal communication skills that will promote team building and care coordination. The Team Lead works collaboratively with other departments to facilitate care coordination across the care continuum. The Team Lead requires collaboration with the Care Coordination Leadership team and oversite of daily case management operations.

$43.00-$56.76hr

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
