St. Peter's Health Partners Community Hospice in the Catskill Region is seeking a Per Diem RN to join the Home Care Team serving Albany and Schenectady counties.
The RN will coordinate with an interdisciplinary team to develop a comprehensive plan of care for patients and families, with weekday hours from 8am to 4:30pm. Requires New York RN licensure, ADN or BSN, and at least 1 year of hospice/home care or acute care experience.
