** Job Description*
* ** LPN For the Office for People with Developmental Disabilities (OPWDD)*
* Western New York DDSOO (DDSOO) is an agency of the Office for People with Developmental Disabilities (OPWDD). The homes are residences of a developmentally disabled population including some individuals who are medically or physically disabled, as well as some who are confined to wheelchairs.  
** There is a
** fluctuating need (one that may change from day-to-day and not for a fixed number of positions).

** LPN Duties to Be Performed*
* ** Consistently attends to health care needs of all individuals*
* + Institutes physicians orders as appropriate and advises residential nurse of new orders for medications and or treatment

+ Prepares administers and documents all medications and treatments prescribed by physicians orders promoting independence based on self medication administration assessments and habilitation plans

+ Administers insulin and monitors blood glucose levels via glucometer administers injectables and medication via tube as well as administering and ensuring accountability of controlled medications. Document as required

+ Follows standard procedure for ordering and receiving medications. Documents using proper form

+ Implement catheterization orders, tracheostomy care, pharyngeal suctioning as well as any additional medical nursing treatment as deemed necessary and within LPN standards of practice

+ Bacuman's health concerns, (including seizures menses vital signs including temperatures BMS unusual behavior etc) Which may indicate health care problems; gathers facts and reports significant information to the appropriate health care providers. Attends medical appointments as assigned

+ Assists staff with concerns or questions regarding individuals care within the LPN scope of practice

** Participates in the developmental and review of Habilitation plans goal plans. Implements and provides documentation for their plans*
* + Provides opportunities for community inclusion utilizing community resources to develop personal relationships with members of the community

+ Provides opportunities to make appropriate choices; offers suggestions for new choices

+ Provides training and assistance to develop skills to promote self reliance

+ Provides opportunities for individuals to contribute to his/her own life and the lives of those around them, both in their community and their home

+ Insurers timely documentation of activities in individual's Habilitation/medical notes

** Promotes individual's safety and rights on a daily basis*
* + Treats individuals with respect and dignity observing their rights to privacy confidentiality and personal choice

+ Provides individuals with a level of supervision which meets their needs as identified in their protective oversight plan and specific issues that may arise

+ Ensures safety by reporting needs repairs safety issues in a timely manner

+ Reports all allegations of neglect mistreatment and abuse promptly and accordingly to established guidelines

+ Maintains current LPM licensure CPR certification and mandated infection control coursework and training

** Consistently adheres to specific duties which are necessary to ensure smooth program operations*
* + Participates in proper meal preparation and portion control according to prescribed diets in individual preferences. Ensures proper food storage following proper sanitation principles

+ Maintains a clean and home like environment

+ Follows personnel and medical policies and procedures; completes necessary documentation

+ Works cooperatively With and communicates appropriately with other team members promoting harmonious environment for the customer

** Performs LPN duties as needed*
* + Administers medication

+ Performs tube feedings

+ Collects blood samples of certified for venipuncture

+ Provides colostomy care

+ takes and records temperature pulse respiration and blood pressure

+ Obtain specimens for analysis and performs other medical procedures as ordered by a physician

+ And applies simple dressing

+ Bathes in feeds patient

+ Cares for individuals in their quarters by performing such tasks as bed making linen change lifting and moving consumers and caring for their personal effects
