You will develop and monitor care plans, supervise aides, and coordinate with primary care and hospital providers to meet resident needs.
The ideal candidate has an NYS RN license, 1–3 years in assisted living or long-term care, and experience with care planning and assessments such as UAS, OASIS, PRI, and Screen.
