The Community Hospice has a wonderful opportunity for a per diem RN to work as part of our Hospice Home Care Team caring for patients and families. Here er's Health Partners, we care for more people in more places. Availability needed for this per diem position would be during the week (Mon-Fri).
Hours generally needed are 8a-4:30p. No weekends or holidays.

What You Will Do:
- Work as part of our Hospice Homecare Team with families residing in the Albany/Rensselaer County region.
- Coordinate with an interdisciplinary team to develop a comprehensive plan of care to provide added support to patients and families in their homes.
- This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
What You Will Need:
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- The RN must be able to communicate effectively, verbally and written
- 1 year experience as an RN , preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
- Understanding of Hospice principles preferred
- Training provided in end of life care.
Pay & Compensation:
Per Diem Rates based on monthly hours commitment
Experienced Pay Range for non-per diem: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
