Location: City of Albany
Trinity Health is seeking a Part-time Registered Nurse for Hospice Homecare/Case Manager in Albany/Rensselaer County. This per diem role typically runs Monday to Friday, 8:00 a.m. to 4:30 p.m., with no weekends or holidays.
Join our Hospice Homecare Team to coordinate care with an interdisciplinary group and support patients and families in their homes. NY RN license, ADN or BSN preferred, and at least 1 year in hospice/home care or acute care required.
