Schedule: Full-Time
Salary per annum: $150,000 - $200,000

We are seeking a friendly and compassionate Endocrinology Nurse Practitioner/Physician Assistant to join our team. As an Endocrinology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues. • Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with Endo conditions and report through our proprietary software system
• You will be cross-trained in other areas of care management
• Work under the supervision of an off-site Collaborative Physician
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes

• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• DEA License
• CPR BCLS/BLS certified
• Must hold a Master’s Degree in Nursing
• Completed a degree from an accredited Nurse Practitioner program
• A year of experience in Endocrinology
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
• Must have great organizational and time management skills.
• Reliable transportation
• Must be willing to travel to different nursing homes within the state of Florida (100% commitment)
