Salary per annum: $150,000 - $200,000

Job SummaryWe are seeking a friendly and compassionate Endocrinology Nurse Practitioner/Physician Assistant to join our team. As an Endocrinology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.
Responsibilities- Assist care coordinators across the different providers and settings
- Promote better health outcomes through communication between facilities and patients
- Assess patients with Endo conditions and report through our proprietary software system
- You will be cross-trained in other areas of care management
- Work under the supervision of an off-site Collaborative Physician
- Gathering and analyzing patient information
- Detailed review of the system
- Review of all pertinent patient labs, studies, and provider notes
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- DEA License
- CPR BCLS/BLS certified
- Must hold a Master’s Degree in Nursing
- Completed a degree from an accredited Nurse Practitioner program
- A year of experience in Endocrinology
- Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
- Must have great organizational and time management skills.
- Reliable transportation
- Must be willing to travel to different nursing homes within the state of Florida (100% commitment)
- Competitive salary per annum: $150,000 - $200,000
