Location: City of Albany
Albany Home Health and Hospice in Albany, OR is seeking a compassionate Licensed Practical Nurse (LPN) to join the On-Call/After Hours Team. This role provides skilled nursing visits and after-hours triage for home health and hospice patients across the Albany area.
You will respond to after-hours calls, administer medications, educate families, document care, and collaborate with physicians and case managers. Oregon LPN license and CPR are required; 1–2 years of acute care experience preferred.
