Location: City of Albany
A government agency is seeking a Social Worker (HUD-VASH Case Manager) to provide intensive case management and clinical services to chronically homeless Veterans. The role involves conducting assessments, developing treatment plans, and coordinating care with interdisciplinary teams. The Social Worker will work closely with community partners to promote reintegration and prevent homelessness. Strong experience in case management and knowledge of housing programs are essential.
This position is based in Albany, NY and is eligible for the Education Debt Reduction Program for student loan reimbursement.
