Residential Clinical & Case Planning Services

Salary range:
$44K-54K
40 hours per week; some evenings and weekend hours to meet the needs of youth and families
Medical, dental, vision, FSA, voluntary life, STD, LTD, Legal Plan, Pet Insurance available the first of the month following thirty (30) days of employment, 403(B), tuition reimbursement, vacation, personal, sick and 11 observed holidays.
Job Description:
Under the direction of the Director of Residential Clinical & Aftercare Services, the case planner will coordinate implementation of treatment plans and act as liaison between St. Anne Institute and family court, placing agencies, families and the community.
Responsibilities:
Provide case planning services to residential and respite youth and aftercare services to youth discharged from the residential program
Maintain regular contact with assigned clients/families, placing agencies and outside service providers
Provide non-clinical and outreach services based on service needs assessment of client caseload including transportation, home visits, parent skills training, assistance with daily living skills, safety planning, supervised visitation and additional services as needed.
Provide orientation to residents and families to agency procedures and programs
Complete Family Assessment Service Plans for each client on caseload according to NYS OCFS standards and time frames
Requirements and
Education:
Bachelor’s degree with 2 years’ experience required; MA, MS or MSW or Masters degree in the field of Human Services preferred; strong verbal and written communication skills required; experience working with diverse populations preferred; valid driver’s license required.
