Family Case Manager
Join us in our mission as a Family Case Manager at Catholic Charities Housing Office. We are committed to making a positive impact in our community.
As the Family Case Manager, you will play a pivotal role in individuals' lives by:
- Completing timely and accurate documentation in the case record including referral intake packets, related information, referrals, correspondences, and entitlement documentation.
- Performing housing office assessments of each new tenant or head of household upon admission and on an annual basis thereafter.
- Developing and implementing Individual Service Plans (every six months), monitoring effectiveness, guiding family members toward positive outcomes, reporting outcomes, and making changes as needed.
- Maintaining case record notes documenting progress toward milestones and performance targets in the ISP.
- Conducting case conferences with the Director of Multi‑Family Housing, CCHO staff, DSS workers, and other treatment providers as needed.
- Cooperating in ongoing supervisory processes with DSS, including attendance at scheduled case management team meetings.
- Attending meetings and advocating on behalf of clients and the program, escorting clients to appointments as required.
Salary: $20.00–$23.00 per hour.
Schedule:
Monday–Friday, 11:00 am–7:00 pm (37.5 hours per week).
- Bachelor’s degree in a related field.
- 1–3 years of experience working with a diverse population.
- Valid driver’s license, proof of current car insurance and registration, and access to a reliable vehicle.
