Salary Range $27.00 - $29.00 Hourly Position Type Full Time Education Level Bachelors Travel Percentage Up to 50% Category Nonprofit - Social Services
DescriptionAs an Intake Specialist, you will communicate with individuals within the community who are seeking OMH residential services. You will gather information, make referrals, and complete paperwork for youth and families coming into program. You will report to the Executive Program Director, Residential and work onsite.
Responsibilities:
- Receive initial phone inquiries and written referrals.
- Gather pertinent information to determine eligibility for services requested.
- Refer to alternate service providers as requested or needed.
- Gather and enter information into database including referral information, demographic data, change of status, etc.
- Meet with clients and families to communicate relevant information regarding services and to receive signatures on releases of information and intake paperwork.
- Prepare and distribute clinical record, admission paperwork and other documents for programs.
- Develop general knowledge of all services to meet the needs of callers, including in emergency situations.
- Schedule intake and admissions in collaboration with family and program.
- Other duties as assigned.
Requirements and
Education:
- Bachelor's degree in human services field.
- Prefer experience in human services field.
- Must pass physical.
- Must be able to complete TCI training.
- Must be able to lift 35 pounds.
