Location: City of Albany
Trinity Health's ALS Regional Center – St. Peter's in the Capital Region seeks a Part-time LMSW/LCSW social worker to provide advanced clinical services, care planning, case management, and resource coordination for ALS patients and families.
The role offers autonomous practice, opportunities for QI participation, and a supportive multidisciplinary team. The MSW is required, NYS licensure preferred, and a minimum of one year in medical/psychiatric/health care settings is desirable.
