POSITION OVERVIEW

This position creates bridges between worlds for students managing mental and behavioral health needs while ensuring the accuracy and continuity of their educational records. The navigation of a successful transition from residential care back to a student's home school and community is a powerful responsibility that requires both strong relationships and clear coordination of educational information. The positive short and long-term impact held within this role is meaningful and rewarding, as it helps ensure students leave the LTCT program with both the support and documentation needed to continue their educational progress.

This position works directly with students and their families, community agencies, and school district staff, administrators, and counselors to support transition planning and reentry into educational programs. The role also maintains and coordinates student enrollment and records processes associated with student transitions, ensuring that receiving school districts have the information needed to support each student's continued success.

Working within a dynamic, creative, positive, and encouraging school community of educators, this role is critical in wrapping students in coordinated services, maintaining accurate educational records, and supporting a smooth transition that allows students to carry forward the progress they have made during their time in the Long Term Care and Treatment program.

QUALIFICATIONS

  • High school diploma or equivalent.
  • Experience working within educational systems serving students with behavioral health needs, special education programs, or alternative education settings preferred.
  • Experience working with student information systems, student records management, and educational documentation preferred.
  • Demonstrated knowledge of state and federal laws governing student records including FERPA, IDEA, and applicable Oregon statutes and administrative rules.
  • Experience working with at-risk students from diverse communities and cultures.
  • Demonstrated ability to collaborate with school districts, families, and community agencies to support student transitions between educational programs.
  • Valid driver's license and reliable transportation.

  • GENERAL INFORMATION

    This position has the following expectations and prerequisite abilities.

  • Maintain a high level of confidentiality in accordance with HIPAA, FERPA and adherence to the competent and ethical educator practices.
  • Maintain compliance with state and federal laws governing all aspects of our agency including ORS, OAR, IDEA, FOIA, and CIPA.
  • Knowledge of rules and regulations applying to students enrolled in Long Term Care and Treatment programs and their reentry into public education.
  • Ability to partner with families, school districts, and community agencies to support student transitions to educational programs following discharge.
  • Exceptional prioritization of tasks, timely completion of work assignments, ability to take initiative and work independently with minimal supervision.
  • Demonstrates respectful, flexible and effective team building skills of cooperation, communication and active participation.
  • Present self and LBL in a professional manner including oral and written communication and professional disposition when working with others.
  • Ability to organize and present student data and records in user-friendly formats for internal reporting and collaboration with school districts.
  • Maintain regular attendance at work and be punctual in meeting deadlines, attending meetings, and following schedules.
  • Utilize teleservice platforms to provide services to educators, students and district staff as directed.
  • Ability to lift, carry at least 25 pounds.
  • Ability to sit for extended periods of time in a vehicle or work setting.
  • Ability to work flexible hours while visiting receiving school districts and community partners, which may include occasional evening travel.
  • Ability to safely maneuver self and others in an unpredictable environment.
