Medical Social Worker (MSW) – Hospice / Home Health | Full-Time | Albany, OR $42.00 – $47.00 per hour + Mileage Reimbursement | Full-Time Make a Meaningful Impact in Hospice Care Signature Albany Home Health & Hospice is seeking a compassionate and dedicated Medical Social Worker (MSW) to join our interdisciplinary Home Heath and Hospice team. This full-time role is ideal for a social worker passionate about end-of-life care, psychosocial support, and patient advocacy.

As a Home Health and Hospice Medical Social Worker, you will provide critical emotional support, care coordination, and community resource guidance to patients and families navigating life’s most meaningful transitions.

What You’ll Do (Key Responsibilities)
Conduct comprehensive psychosocial assessments for hospice patients and families

Develop and implement care plans based on social and emotional needs

Provide grief counseling, bereavement support, and crisis intervention

Coordinate community resources, referrals, and support services

Serve as a liaison between patients, families, and healthcare providers

Collaborate with the interdisciplinary hospice team (RN, physician, aides)
Educate caregivers on coping strategies and end-of-life planning

Participate in quality assurance and performance improvement initiatives What You Bring Bachelor’s or Master’s degree in Social Work (MSW or BSW) from an accredited program1+ year of healthcare experience (hospice or home health preferred)
Understanding of hospice philosophy, end-of-life care, and grief processes

Strong communication, organization, and interpersonal skills

Ability to work independently in a home health / field-based setting

Current CPR certification

Valid driver’s license and reliable transportation
