Role OverviewWe're hiring a compassionate Occupational Therapist to deliver high-quality, in-home care. You'll help patients improve independence, function, and quality of life while collaborating with a supportive care team.
What You'll DoAssess patients and create individualized care plans Provide therapy based on physician orders Educate patients, families, and caregivers Evaluate home safety and recommend equipment Track progress and maintain clinical documentation Collaborate with physicians and care teams Train patients in daily living skills & adaptive equipment Supervise COTAs and home health aides (as applicable) Support discharge planning and continuity of care
QualificationsLicensed Occupational Therapist (state required) Accredited OT degree Home health experience preferred Strong communication & independent work skills Valid driver's license & reliable transportation
