What You'll DoEvaluate patients and create individualized treatment plans

Deliver skilled physical therapy services in the home setting

Partner with physicians, nurses, and interdisciplinary team members

Educate patients and caregivers on treatment goals and home exercise programs

Complete timely and accurate clinical documentation

What You'll Bring Current Oregon Physical Therapist License Valid driver's license and reliable transportation

Home health experience preferred, but not required

Strong communication and clinical skillsA passion for helping patients achieve their goals
