Hospice/Medical Social Worker
Days (08:00 AM - 04:30 PM), no weekends
The Hospice teams provide essential support to patients and their family members during the final stages of illness. Services are delivered in a variety of settings, including patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Offered services include:
- Intermittent skilled nursing visits
- Pain and symptom management
- Medical social services
- Counseling and spiritual support
- Hospice aides
- Trained hospice volunteers
- Bereavement support
- Medications and equipment as appropriate
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
- Healthcare Provider Level BLS certification required
- Valid driver's license and auto liability/property damage insurance, with maintenance of vehicle in good working order
- Oregon LCSW (Licensed Clinical Social Worker) preferred
- Experience as a social worker in a hospital or medical setting is preferred
Holiday coverage is required for this position.
