Albany Home Health & Hospice is seeking an experienced and compassionate Start of Care Registered Nurse (SOC RN) to join our growing clinical team. This full-time position is ideal for an RN who excels in patient assessment, care planning, and care coordination while delivering exceptional home health and hospice services.
As a Start of Care RN, you will be the first clinical connection for patients beginning their home health or hospice journey. Key Responsibilities
- Complete comprehensive Start of Care (SOC) assessments in patients' homes.
- Perform OASIS assessments and admission documentation accurately and timely.
- Develop individualized plans of care based on physician orders, patient goals, and clinical findings.
- Complete medication reconciliation and review.
- Establish emergency preparedness plans and patient-specific care strategies.
- Obtain all required consents and admission documentation.
Clinical Care & Coordination
- Coordinate admissions with Intake, Clinical Managers, physicians, and interdisciplinary team members.
- Communicate patient needs, changes in condition, and recommendations to providers.
- Arrange for necessary equipment, supplies, and support services.
- Provide patient and caregiver education regarding diagnoses, medications, treatments, and self-care.
Documentation & Compliance
- Complete admission documentation within required timelines.
- Maintain accurate, thorough, and compliant clinical records.
- Ensure compliance with Medicare, state, federal, and agency regulations.
- Document patient progress, symptom management, and outcomes.
Team Collaboration
- Participate in care conferences, quality improvement activities, and interdisciplinary meetings.
- Assist with orientation and mentorship of new clinicians as assigned.
- Support agency operations through skilled nursing visits and RN case management as needed.
Qualifications
- Graduate of an accredited School of Nursing.
- Current Oregon Registered Nurse (RN) license.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently and manage multiple priorities.
Preferred
- 1–2 years of recent home health and or hospice experience.
- Experience performing OASIS assessments and Start of Care admissions.
- Knowledge of Medicare home health and hospice regulations and documentation requirements.
- Acute care, community health, or hospice experience.
Physical Requirements
- Ability to stand, walk, bend, kneel, and reach frequently.
- Ability to lift, transfer, and position patients safely.
- Ability to transport clinical equipment and supplies.
- Ability to travel throughout the service area and work in various home environments.
