• Provide home‑based care to seniors and patients with illness, injury, or chronic disease.
• Assist with bathing, toileting, dressing, grooming, nail/skin care, eating, and nutrition.
• Help with ambulation, transfers, and range‑of‑motion exercises.
• Check vitals, record intake/output, monitor blood glucose, and document changes.
• Observe patient status and notify supervisor of any issues needing changes in care, medication, or equipment.

• High school diploma (or equivalent) and an active Nursing Assistant certification in Oregon.
• CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Compassionate nature, good communication skills, and ability to lift up to 50 lbs.
