In this role, you will assess, diagnose, and manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, and develop evidence-based care plans. You will collaborate closely with physicians, nurses, and interdisciplinary teams to deliver patient-centered, culturally competent care. Responsibilities include patient education, care coordination, documentation in the EHR, quality and safety initiatives, and contributing to a supportive, learning-focused clinic environment that reflects Legacy's community health mission.
