South Hills Rehabilitation Center in Eugene is looking for dedicated LPNs to join our supportive nursing team.

South Hills Rehabilitation Center is part of the Volare Health family of long-term care skilled nursing and short-term rehabilitation centers. We believe in caring for our residents, supporting their families, and investing in the people who provide that care.
What You'll DoAs an LPN, you will play an important role in providing safe, compassionate, and individualized care to our residents.
Responsibilities include:
Assessing resident needs and monitoring changes in condition
Developing and implementing individualized care plans
Providing direct nursing care and ensuring residents' needs are met
Supervising CNAs and other care team members in the delivery of nursing services
Maintaining accurate and timely clinical documentation
Following applicable state and federal regulations, facility policies, and nursing standards
Communicating effectively with residents, families, providers, and the interdisciplinary care team
Advocating for residents and helping promote their comfort, dignity, and quality of life
Current, active Licensed Practical Nurse (LPN) license in the state of Oregon
Ability to assess resident needs and respond appropriately to changes in condition
Knowledge of individualized care planning and nursing practices
Ability to supervise and work collaboratively with CNAs and other healthcare team members
Strong communication and organizational skills
Compassion, dependability, and a genuine desire to make a difference in the lives of others
Commitment to following state and federal regulations governing skilled nursing care
