Registered Nurse (RN) - Home Health
About the Facility
- Home Health Services based at 901 N. 2nd St., Albemarle
- Community-based care model serving multiple surrounding areas
- Radius Rule:
Candidates must live 50+ miles away from the facility
We are seeking a skilled Home Health Registered Nurse (RN) to provide in-home patient care across the Albemarle region. This role focuses on independent patient management, clinical assessments, and care coordination in a fast-paced, field-based environment.
Territory Coverage
- Albemarle
- Badin
- Harrisburg
- Mt. Gilead
- Norwood
- Troy
- Surrounding communities
- Average 60-80 miles per day
- Mileage reimbursed weekly
- No maximum mileage cap
- Expected 6 visits per day
- 30 Visit Equivalents (VE) per week required
- Visit Types Include:
- Routine visits
- Start of Care (SOC)
- Resumption of Care (ROC)
- Recertifications
- Reassessments
- Discharges
Patient Population
- Chronic disease management
- Wound care (including wound vac)
- IV therapy
- Pleur
X catheter care - General home health cases requiring education and monitoring
- Minimum 1 year of Home Health RN experience
- First-time travelers:
Accepted (must be experienced in home health)
- Active RN License
- BLS (Basic Life Support)
- Valid Driver's License
- Proof of Auto Insurance
- Homecare Homebase (HCHB) experience - REQUIRED (no exceptions)
- OASIS documentation
- Case management (handling 25-30 patients simultaneously )
- IV therapy, wound care, pleurx, wound vac
- Patient assessment and disease education
- Ability to work independently and manage schedule
- EMR:
Homecare Homebase (HCHB) - Required - EPIC Rover - view-only access
- Focura - documentation uploads (e.g., wound images, discharge instructions)
- Company-issued phone:
Samsung device for documentation and communication
- Schedule Cycle: 13-week contract
- Weekend Requirement:
None (optional if interested) - On-Call:
Not required (optional opportunities available) - Holiday Requirement:
Must work 75% of holidays - RTO:
Requires manager approval
- Nurses schedule visits directly with patients the day prior
