RN Care Manager – Part-Time / 24-Hour Weekend Schedule
Midwest rural community provider Atrium Health Stanly – Case Management seeks an RN Care Manager to coordinate and optimize patient care across multiple settings. The position operates a 24‑hour weekend schedule (Saturday and Sunday 7am‑7pm) with a pay range of $38.20–$57.30 per hour.
Essential Functions- Utilize the RN process framework to guide the healthcare team toward optimal outcomes and efficient resource use.
- Collaborate with patients, professionals, and the community to ensure continuity of care and coordinate services across care settings.
- Conduct comprehensive all‑system needs assessments and coordinate disease‑management and health‑maintenance services for identified populations.
- Develop and maintain accurate case records for each referred patient.
- Document care plans fully and accurately using the organization’s computer databases and documentation systems.
- Maintain knowledge of reimbursement criteria and necessary documentation for Medicaid, Medicare, and managed care.
- Demonstrate nursing leadership, evaluating personal practice against professional standards, statutes, rules, and regulations.
Work requires walking, standing, lifting (minimum 35 pounds at shoulder height), reaching, bending, and stooping. Ability to travel between locations is required. Frequent verbal and written communication in English is essential. The role also requires visual, auditory, and manual dexterity, critical thinking, and the ability to respond quickly to changing assignments. Exposure to intermittent noise, dust, fumes, and chemicals may occur.
Education,Experience, and Certifications
BSN is required; a master’s degree is preferred. Current RN license or temporary license is required, or an accepted national license. A minimum of two years of health‑care experience is required, including case management or discharge planning experience in acute care, home care, LTC, physician office, or managed‑care settings. Professional certification within three years of hire is required per departmental protocol.
BLS certification is required.
Base compensation is within the listed pay range, with opportunities for shift, on‑call, and other premium pay. Incentive pay may be available for select positions, and annual increases are considered based on performance.
