Home Health Nurse
Conduct comprehensive patient assessments and develop individualized care plans.
Manage an average of 6 patient visits per day, including routine visits, discharges, Start of Care (SOC), Resumption of Care (ROC), recertifications, and reassessments.
Provide disease-specific education to patients and caregivers.
Perform wound care, wound vac management, IV therapy, and PleurX catheter care.
Complete OASIS documentation accurately and promptly.
Oversee episode management and case management for a caseload of approximately 25-30 patients.
Coordinate care with physicians, therapists, and interdisciplinary healthcare teams.
Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.
Maintain accurate documentation using Homecare Homebase (HCHB).
Active RN License.
Minimum of 1 year of recent Home Health nursing experience.
Homecare Homebase (HCHB) experience required - no exceptions.
Current BLS certification.
Valid Driver's License and proof of auto insurance.
Previous traveler experience is a plus; however, first-time travelers will be considered if they are seasoned Home Health nurses.

Average daily travel is approximately 60-80 miles, with mileage reimbursement provided weekly.
