American Traveler is seeking an RN for a Home Health position requiring 1 year of home health experience and mandatory Homecare Homebase (HCHB) EMR experience.

• Home health nursing services delivered in patients' homes throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas
• 8-hour day shift schedule, , 40 hours per week
• Caseload includes chronic disease management, wound care, IV therapy, pleurx drains, and wound vac patients
• Responsible for 6 visits per day including routine visits, discharge, ROC, recertification, and reassessment visits
• Case management responsibilities for up to 25-30 patients at once
• Average daily travel of 60-80 miles with no maximum mileage cap; mileage reimbursed weekly
• No maximum mileage limit for travel between patient visits
• Uses HCHB (Homecare Homebase) as primary EMR, with EPIC Rover for view-only access
• Utilizes Focura for uploading discharge instructions, wound photos, and other documentation
• Company-provided phone used for documentation and communication with patients and providers
• No weekend rotation required unless interested in working weekends
• On-call not required but may be available for those interested
• 13-week schedule cycle

• Active RN license (NC or Compact) required
• BLS certification required
• Minimum 1 year of home health experience required
• Homecare Homebase (HCHB) EMR experience required with no exceptions
• Valid driver's license and proof of insurance required
• Skills required include IV therapy, pleurx management, wound vac care, wound care, patient assessment, disease teaching, OASIS, and case/episode management
• Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN
• Last 4 digits of SSN and full date of birth required for consideration

• Schedule patient visits one day in advance of planned visits
• Required to complete 30 visit equivalents (VE) per week; cancelled appointments do not count toward VE totals
• Supported by departmental leadership, a scheduler, Clinical Manager, Clinical Supervisor, and clinical peer team
• Orientation includes NEO (New Employee Orientation) and 5 days of facility/unit orientation
• Expected to work 75% of holidays
• RTO requests subject to manager approval
• Navy blue scrubs required
• Free parking available

• First-time travelers welcome if they have seasoned home health experience
• Must reside at least 50 miles outside of home unit location
• Candidates previously employed in a permanent position with this health system must have been separated for at least 1 year before eligibility
