As a Wound Care Nurse Practitioner, you'll work directly inside assisted living communities, memory care settings, and long‑term care environments throughout the Twin Cities. Every day brings different patients, different challenges, and different opportunities to use your clinical judgment.

- Provide advanced wound care assessments, treatment planning, and ongoing management.
- Conduct routine rounding visits throughout assisted living, memory care, and long‑term care communities.
- Manage medications, diagnostics, treatment plans, and follow‑up care.
- Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
- Support chronic disease management, preventative care initiatives, and continuity of care.
- Help improve healing outcomes while reducing avoidable complications and hospitalizations.
- Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
- Experience in wound care, geriatrics, primary care, post‑acute care, skilled nursing, or related settings preferred.
- Strong clinical judgment and confidence managing medically complex patients.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
