Job Title: RN Case Manager – Dignity. Compassion. Clinical Excellence.
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area, 21-County Service Area (territories assigned based on coverage needs)
Responsibilities- Support patients through complex physical and emotional transitions.
- Provide clear, compassionate education and reassurance to families.
- Coordinate a seamless end‑of‑life care experience from admission forward.
- Manage pain, symptoms, and comfort‑focused care plans.
- Collaborate with social workers, aides, chaplains, and medical directors.
- Maintain accurate, timely documentation to support high‑quality care.
- Holds an active Minnesota Registered Nurse license.
- Has experience in hospice, oncology, or palliative care.
- Balances clinical skill with heart‑centered care.
- Communicates with empathy, clarity, and confidence.
- Understands the sacred nature of end‑of‑life care and approaches it with grace.
Military medical experience is highly valued. We welcome veterans who bring leadership, resilience, and compassion to patient care.
