- Under general direction from supervisor; responsible for the intake, interview and assessment of inmates in accordance with pertinent Federal, State laws and departmental policies and procedures.
- Responsible for the review of inmate history and records to determine eligibility for social services benefits/programs including, but not limited to: housing, shelters, treatment, food stamps, Medicaid, discharge planning, medical service centers, community custody and other resources.
- Maintain confidentiality and knowledge of Federal and State laws and regulations in carrying out assignments.
- This is a correctional facilities work environment which requires direct interactions with inmates at various levels of classification.
- Conduct case management services for assigned inmates; assess inmates' needs including, but not limited to: drug and alcohol abuse, domestic violence, behavioral health and reentry resources.
- Act as key point of contact for assigned units/inmates to assist in enrollment for benefits/eligibility programs.
- Conduct initial interviews with inmates; review prior detention history; review documents from other facilities; prepare, maintain and update inmate files; coordinate discharge planning and processing of social services documentation.
- Ensure timely and accurate documentation and reporting of case management processes; maintain case notes and related documentation.
- Create and maintain partnerships to facilitate effective social services initiatives and outcomes.
- Serve as liaison with community benefit providers and MDC.
- Monitor program compliance with laws, rules and regulations related to case management services and related SOPs.
- If assigned to the Community Custody Program (CCP), act as primary liaison and facilitate classification to CCP by court order or review process; ensure timely and accurate follow-up on all communication, reporting and recording requirements related to CCP.
- Perform other job-related duties as required or assigned.
- Bachelor Degree in related field, plus three (3) years of work experience in criminal justice, social work, counseling, case management or related field; OR seven (7) years of related education/experience in lieu of degree.
- Knowledge of basic operations, services and activities of a correctional program, program development/implementation, and court system operations.
- Knowledge of community social services and benefits available to inmates reentering the community.
- Knowledge of counseling and case management techniques.
- Knowledge of methods and techniques for determining security level and inmates' rights principles and practices.
- Skill in using computers to generate reports and maintain records.
- Ability to interview and evaluate inmates for program eligibility and interpret court documents.
- Ability to communicate clearly in English, both orally and in writing.
- Ability to work a flexible schedule including evenings and weekends.
- Ability to analyze hazardous situations rationally and remain calm under stress.
- Ability to interact professionally with administration, staff, inmates and the public.
- Ability to analyze problems, identify solutions, and implement recommendations in support of goals.
- Ability to establish and maintain effective relationships with staff, County officials and the public.
- Employee must successfully complete a post-offer medical examination and background investigation.
- This position is safety sensitive and subject to pre-employment and random drug testing.
- Employee must comply with Bernalillo County safety policy requirements.
- Employee must complete specified trainings within nine months of hire: NIC Evidence Based Practices Course #4, and Medicaid Presumptive Eligibility training through the State of New Mexico.
- Most duties are indoors in a temperature-controlled environment; outdoor duties occur and exposure to weather is possible.
- Work hazards include those of a correctional facility environment with potential exposure to noise and dangerous situations.
- Exposure to intermittent noise and vibration, fumes and odors, contagious diseases; potentially hostile scenarios.
- Indoor surfaces may be carpeted or tiled; stairs may be involved.
- Outdoor surfaces may be even or uneven and may be wet or dry; training and work may require stairs or ladders on inclines or hills.
- Flexible schedule may be required.
- Administrative tools include telephone, computer, fax, photocopier and pager.
- May handle first aid and oxygen equipment occasionally.
- Various forms, reports and office supplies are used in duties and responsibilities.
