Start Date: 12-01-2025

End Date: 03-02-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM Day shift

Hours per Week: 36

Roles & Responsibilities
  • Conduct thorough speech-language evaluations to create clear, patient-centered plans
  • Deliver daily treatments that improve communication, swallowing, and quality of life
  • Coordinate and manage safe discharges, ensuring continuity of care
  • Document clinical progress and maintain productivity expectations (90%) with accuracy
  • Collaborate closely with the interdisciplinary team to deliver holistic patient care
  • Represent the therapy team professionally (black scrubs dress code) and support facility operations
Qualifications
  • Active New Mexico SLP license or eligibility to obtain state licensure
  • 1-3 years of clinical experience; prior SNF experience strongly preferred
  • Master's degree in Speech-Language Pathology (or equivalent clinical graduate degree)
  • ASHA CCC-SLP preferred but not required
  • Ability to follow Continuum Rehabilitation Group policies and productivity standards
  • Willingness to be onsite to receive guaranteed hours and adhere to dress code
Education
  • Master's degree in Speech-Language Pathology
Knowledge & Skills
  • Skilled in evaluations, daily treatments, and discharge planning
  • Experience with SNF workflows, documentation, and productivity tracking
  • Strong clinical reasoning, time management, and patient-family communication
  • Comfort working within an interdisciplinary therapy team
  • Ability to adapt treatment plans for geriatric and medically-complex patients
