About the Opportunity

UNM Medical Group, Inc. is hiring a Community Health Worker for a full‑time day shift position at the Truman Health Services Clinic in Albuquerque, New Mexico. The role is a term position ending 9.30.2028.

  • Minimum salary $44,604 – midpoint $55,766; salary based on relevant experience; 40‑hour full‑time equivalent with prorated hours for less than 40

Summary

Under general supervision and as part of a multidisciplinary team, the Community Health Worker improves retention in care for specialized patient subsets. Responsibilities include building patient relationships, conducting prevention interventions, presenting health information related to chronic disease, and performing necessary administrative functions to ensure efficient program operation.

Minimum

Job Requirements
  • High School diploma or GED
  • Complete New Mexico State CHW Certification within one year of employment
  • Accredited degree may substitute for experience on a year‑for‑year basis
  • Verification of education and licensure required if selected for hire
Required License
  • Valid unrestricted New Mexico driver’s license
Duties and Responsibilities
  • Assist clients in homes, community, or clinic settings, communicating program purposes and impacts
  • Help patients identify socio‑economic issues, develop health/social management plans and goals
  • Identify individuals who would benefit from retention in care strategies
  • Coach patients in managing chronic conditions and self‑care; motivate active engagement
  • Assist clients in overcoming barriers to medical care and social services
  • Provide support and advocacy during initial medical visits or when necessary to convey medical needs and referrals
  • Build positive relationships with clients, providers, nurse case managers, agency representatives, supervisors, and office staff, reducing cultural and socio‑economic barriers
  • Participate in community events, health fairs, and school events to promote retention in care clinics
  • Provide accurate health information and referrals with respect, active listening, empathy, and patience
  • Maintain professional growth through required departmental competencies
  • Perform miscellaneous job‑related duties as assigned
