As a Medical Social Worker, you help patients and families navigate moments of uncertainty with clarity, compassion, and dignity.

- Provide social work services under a physician’s plan of care, supporting patients’ emotional, social, and environmental needs.
- Assess psychosocial factors affecting care and help families access financial, community, and supportive resources.
- Partner closely with the interdisciplinary team to develop individualized care plans and ensure smooth care transitions.
- Support patients and families as they navigate complex family dynamics, offering clarity and steadiness during difficult conversations.
- Maintain timely documentation, participate in care conferences, and uphold all Medicare, state, and professional standards.
- Master’s in Social Work (MSW) from a CSWE-accredited program; we welcome newer graduates with strong clinical rotations.
- Empathetic communicators who bring calm, clarity, and confidence to emotionally complex situations.
- Individuals skilled in navigating difficult conversations and building trust with patients and families.
- Self-directed, reliable professionals who thrive in field-based work and value collaboration with clinical partners.
- Valid driver’s license, auto insurance, and comfort traveling locally in varied conditions.
