- Perform thorough patient evaluations to identify functional goals and develop individualized treatment plans
- Deliver evidence-based therapeutic treatments and monitor patient progress daily
- Plan and coordinate safe, efficient discharges that prioritize rehab-to-home outcomes
- Document patient status, treatment, and progress accurately and timely
- Collaborate with the interdisciplinary rehab team to optimize patient care and transitions
- Meet productivity expectations and contribute to a high-performing therapy program (facility requires 90% productivity)
- Follow Continuum Rehabilitation Group (CRG) policies and procedures to maintain care standards
- Be present on-site to receive guaranteed hours and support the facility’s rehab census and workflow
- PT or DPT credential (DPT strongly represented in this role)
- 1-3 years of experience (as listed: 1-3 years); prior SNF experience strongly preferred
- Active, state-appropriate Physical Therapist licensure (ability to work in New Mexico preferred/required per facility rules)
- Comfortable working in a Skilled Nursing Facility (SNF) setting with a high rehab-to-home population
- Ability to meet productivity expectations and follow CRG policies & procedures
- Professional, punctual, and able to be present in the building to receive guaranteed hours
- Willingness to wear required dress code (black scrubs) while on assignment
- Doctor of Physical Therapy (DPT) or equivalent physical therapy degree
- Skilled in patient evaluations and goal-driven treatment planning
- Experience delivering therapeutic treatments across common SNF diagnoses
- Knowledge of discharge planning and transitions to home or higher levels of care
- Familiarity with SNF documentation standards and productivity tracking
- Strong communication and teamwork skills within a rehab interdisciplinary team
- Ability to prioritize caseloads and manage daily treatment schedules efficiently
