Travel Physical Therapist SNF

We’re seeking a motivated and compassionate Travel Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing / Post-Acute Care setting
. This role offers the opportunity to work with a geriatric population, collaborate with an interdisciplinary rehab team, and make a meaningful impact on patient mobility and independence.

  ASAP or upcoming start dates available

• Evaluate patients and develop individualized plans of care
• Provide skilled PT interventions focused on strength, balance, gait, and functional mobility
• Monitor patient progress and adjust treatment plans as needed
• Accurately document evaluations, treatments, and outcomes
• Collaborate with nursing and rehabilitation teams
• Educate patients and caregivers on mobility, safety, and home exercise programs

• PT or DPT from an accredited program
• Active or eligible Physical Therapist license
• SNF or post-acute experience preferred (new grads may be considered, depending on facility)
• Strong communication, time-management, and teamwork skills
• Commitment to patient-centered care
