About the Position
Occupational Therapist (OT) – Skilled Nursing Facility (SNF)
We are   seeking   a skilled and compassionate     Occupational Therapist (OT)     to join a collaborative therapy team within a skilled nursing facility setting. This position focuses on providing high-quality rehabilitation services to a primarily geriatric population, including short-term rehab patients and long-term care residents.
The OT will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions designed to restore functional independence, improve safety, and enhance overall quality of life. This role requires strong clinical judgment, efficiency, and the ability to thrive in a fast-paced, productivity-driven environment.

Key Responsibilities:

Perform comprehensive occupational therapy evaluations and   establish   individualized plans of care

Provide skilled treatment targeting ADLs, functional mobility, strength, balance, cognition, and safety awareness

Implement fall prevention strategies and environmental modifications

Recommend and train residents in adaptive equipment and compensatory techniques

Complete   accurate   and   timely   documentation, including evaluations, progress notes, recerts, and discharge summaries

Collaborate with interdisciplinary team members including PT, SLP, nursing, and case management

Supervise and direct COTAs   in accordance with   state practice guidelines

Maintain productivity standards while ensuring patient-centered care

Ideal Candidate:
Active OT license in practicing state

SNF or geriatric experience preferred

Strong clinical reasoning and time management skills

Comfortable managing both short-term rehab and long-term care caseloads
