Key Responsibilities- Evaluate speech, language, cognitive‑communication, and swallowing disorders in home‑based patients
- Develop individualized treatment plans based on physician orders and clinical assessments
- Provide direct speech therapy services to help patients regain or improve communication and swallowing abilities
- Educate patients and family members on exercises and strategies for ongoing progress
- Document patient progress, update care plans, and coordinate with interdisciplinary team members
- Participate in care conferences, in‑service training, and performance improvement activities
- Maintain compliance with HIPAA and company policies for patient safety and confidentiality
- Master’s degree in Speech‑Language Pathology
- Current SLP license in the state of New Mexico
- Certificate of Clinical Competence (CCC‑SLP) from ASHA
- Prior experience in home health or medical settings preferred
- Strong interpersonal and organizational skills
- Self‑starter who thrives in an independent, mobile care environment
- Valid driver’s license and reliable transportation
