• Portneuf Medical Center is a nationally recognized healthcare leader with 205 beds, serving as the region's tertiary care and Level II Trauma Center, housing the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue.

To assure clinical appropriateness of services and efficient use of resources through collaboration with the entire health care team for acute care patients. The Case Manager is responsible for medical necessity review, discharge planning, and utilization review.

• Performs medical necessity reviews and initial assessment within 24 hrs of patient admission.
• Identifies and communicates changes in the patients plan of care to the health care team.
• Maintains detailed knowledge of community resources and process for referals.
• Completes all assigned UR tasks on a daily basis.
• Using LOS anticipates patient needs, stewardship of resources and timely transitioning of patients to alternative level of care.
• Documents in patient record in a consistent and timely manner to include initial screenings and discharge plan.

Job Requirements:

• Current RN license
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Job Requirements:

• Minimum of 5 years' experience, preferably in a hospital setting, with case management certification.
