The Assisted Outpatient Treatment (AOT) Supervisor/Coordinator is responsible for the administrative coordination, operational oversight, and management of AOT services within the Assertive Community Treatment (ACT) program. The AOT Supervisor/Coordinator ensures compliance with court-ordered treatment requirements, oversees referral and enrollment processes, coordinates client services, facilitates treatment planning, and monitors program performance related to AOT participants. This position serves as a primary liaison between the ACT team, courts, hospitals, community providers, and referral sources to promote timely access to services and adherence to treatment requirements.

The AOT Supervisor/Coordinator provides leadership in the administration of the AOT program through oversight of workflows, referral management, quality monitoring, and program operations. While this position exercises administrative authority and operational decision-making responsibilities, it does not provide clinical supervision. The AOT Supervisor/Coordinator works collaboratively with the ACT Team Lead and multidisciplinary team to ensure effective service delivery, regulatory compliance, and positive client outcomes.

Essential Duties and Responsibilities Program Management and Operational Oversight Provide administrative oversight of AOT program operations and workflows.

Develop, implement, and monitor processes that support efficient service delivery and regulatory compliance.

Exercise decision-making authority regarding referral acceptance, program enrollment, and service coordination in accordance with agency policies and program requirements.

Monitor program performance, service delivery outcomes, and adherence to contractual obligations.

Track key performance indicators and identify opportunities for process improvement.

Recommend and implement workflow enhancements to improve client engagement, access to care, and program effectiveness.

Serve as a subject matter expert regarding AOT policies, procedures, and court-ordered treatment requirements.

Referral and Intake Management Review, evaluate, and process incoming AOT referrals.

Determine program appropriateness and eligibility based on established criteria and contractual requirements.

Coordinate intake, enrollment, and onboarding activities for new participants.

Maintain oversight of referral tracking systems and ensure timely processing of referrals.

Collaborate with hospitals, courts, crisis providers, and community partners to facilitate referrals and transitions of care.

Monitor referral trends and provide recommendations regarding program capacity and service needs.

Court Compliance and Reporting Complete, review, and submit AOT adherence reports and related documentation to the court within required time frames.

Monitor participant compliance with court-ordered treatment plans and service participation requirements.

Maintain accurate records and documentation related to enrollment, treatment adherence, and program outcomes.

Coordinate communication with court personnel, legal representatives, hospitals, and other stakeholders as appropriate.

Ensure all court-related reporting requirements are completed accurately and on schedule.

Treatment Planning and Service Coordination Develop, review, and update individualized treatment plans in collaboration with clients and the ACT team.

Ensure treatment plans are person-centered, recovery-oriented, and compliant with agency, payer, and regulatory requirements.

Monitor treatment progress and coordinate plan revisions as needed.

Assist in identifying and addressing barriers to treatment engagement and service access.

Support continuity of care through collaboration with providers and community partners.

Appointment and Care Coordination Oversee scheduling and coordination of psychiatric, medical, behavioral health, and supportive service appointments.

Monitor appointment completion and follow-up activities to support treatment adherence.

Coordinate communication among providers to ensure continuity and integration of services.

Ensure participants receive timely access to required services and supports.

Meeting Facilitation and Team Leadership Plan, facilitate, and document AOT coordination meetings twice weekly.

Develop agendas, review participant status, and track action items to completion.

Coordinate multidisciplinary discussions regarding treatment adherence, referrals, court reporting requirements, and service needs.
