The Care Team Lead is responsible for the day-to-day leadership, oversight, and development of an assigned team of Patient Coordinators. Reporting to the Care Team Director, this role ensures the consistent execution of care coordination processes, promotes operational excellence, and supports the delivery of exceptional patient and caregiver experiences.

The Care Team Lead provides daily coaching, guidance, and accountability to Patient Coordinators while monitoring productivity, quality, documentation, and service standards. This position serves as the first point of escalation for routine operational concerns and partners closely with the Care Team Director to achieve departmental goals and drive continuous improvement. #INDNM/ABQ

Responsibilities:

1. Team Leadership:

  • Provide direct supervision, coaching, and support to assigned Patient Coordinators.
  • Foster a positive, collaborative, and accountable team environment that reflects Giving Home Health Care's HEART Values.
  • Establish daily priorities and ensure workloads are balanced to meet patient and operational needs.
  • Conduct regular one-on-one meetings, coaching sessions, and performance discussions.
  • Assist with onboarding and mentoring new Patient Coordinators.
  • Recognize employee achievements while addressing performance concerns in a timely and constructive manner.
  • Escalate complex operational, employee, or patient concerns to the Care Team Director when appropriate.

2. Operational Oversight:

  • Monitor daily workflows to ensure timely completion of patient care coordination activities.
  • Assist in assigning caseloads and adjusting workloads based on staffing needs and business demands.
  • Ensure timely completion of patient documentation, Letters of Medical Necessity (LOMNs), care plans, and other required records.
  • Identify workflow barriers and implement solutions to improve efficiency and patient experience.
  • Maintain compliance with organizational policies, regulatory requirements, and departmental procedures.

3. Training and Employee Development:

  • Provide ongoing coaching, education, and hands-on training to support employee growth and competency.
  • Reinforce standard operating procedures and ensure consistent application across the team.
  • Identify training opportunities and partner with the Care Team Director to implement development plans.
  • Support cross-training initiatives to improve team flexibility and operational continuity.

4. Quality Assurance:

  • Perform routine quality audits of documentation, medical records, and care coordination activities.
  • Monitor compliance with quality standards, productivity expectations, and regulatory requirements.
  • Provide timely feedback and corrective coaching to improve accuracy and consistency.
  • Assist with quality improvement initiatives and monitor action plans.

5. Performance Management:

  • Monitor team productivity, quality metrics, and service standards.
  • Utilize operational reports and performance dashboards to identify trends and opportunities for improvement.
  • Assist the Care Team Director in achieving departmental goals and key performance indicators.
  • Promote accountability while fostering a culture of continuous improvement.

6. Collaboration and Communication:

  • Serve as the primary resource for Patient Coordinators regarding daily operational questions.
  • Collaborate with Clinical Services, Staffing, Recruiting, Compliance, Authorization Specialists, and other departments to support patient care.
