SummaryThe CNS/NP serves as a clinical resource and independent practitioner who diagnoses and manages acute and chronic diseases on the inpatient unit, proactively manages patient care across the continuum, and oversees case management of complex patients.
Responsibilities- Completes a comprehensive health assessment, including health history and physical exam.
- Determines the need for and interprets screening and diagnostic tests.
- Uses sound clinical judgment to formulate diagnosis by critically analyzing history, exam, and tests.
- Adapts assessment techniques according to patient condition, culture, age, and developmental stage.
- Communicates findings, prognosis, and treatment options; establishes a mutually acceptable plan of care.
- Manages acute and chronic conditions with pharmacologic and non‑pharmacologic interventions, assists patients in following treatment plans, and selects best practice treatments.
- Prepares written prescription orders, medications, and controlled substances per state and federal regulations.
- Initiates timely consultation, referral, or collaboration with the hospitalist and other team members.
- Implements health promotion strategies tailored to age and cultural considerations to reduce risk and prevent disease.
- Demonstrates professional rapport with patients, families, and team members.
- Maintains patient medical records as required by hospital and regulatory agencies.
- Performs other duties as assigned.
Skills and Qualifications
Minimum two to three years experience in an acute care setting required; prior hospitalist experience desirable. Knowledge of medical practice, policies, procedures, equipment, safety hazards, and record‑keeping. Ability to react calmly to emergency situations.
