Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends)! The position is a full‑time (40 hours per week) Psychiatric Advanced Practice Provider for a float role between our centers located in Albuquerque, NM. This role can be filled by a Nurse Practitioner or Physician Assistant. Prior APP experience in a psychiatric/mental health setting is required.
The base salary range is $120,000 – $130,000, plus a no‑cap incentive compensation bonus program.

The Advanced Practice Provider (NP/PA) provides mental health primary and urgent care services to nursing center residents in collaboration with other medical providers in the center. The APP works collaboratively with Center Leadership to achieve the facility operator mission of improving patient care through high‑quality health care and everyday compassion.
Responsibilities- Utilizes the scientific process and national standards of care as a framework for managing patient care, including assessment of health status, diagnosis, interpretation of labs and diagnostics, and psychiatric evaluations to determine mental health needs.
- Manages ongoing medication therapy, following evidence‑based practice guidelines.
- Provides psychotherapy for the treatment of conditions.
- Develops and implements the treatment plan in collaboration with the interdisciplinary care planning process.
- Monitors and evaluates patient progress, adjusting treatment plans as necessary.
- Completes all necessary documentation regarding patient health care in accordance with policies, procedures, and relevant state and federal regulatory requirements.
- Educates patients and families about mental health conditions, treatment options, and self‑care strategies.
- Consults with a physician when patient needs fall outside the scope of practice.
- Works with Align Med leadership and area market leadership to improve quality measures related to psychiatric care.
- Maintains continued professional competence through continuing education, certification maintenance, and application of standardized care guidelines.
- Supports the development of nursing staff, collaborating with the Center Nursing Executive on clinical education priorities.
- Serves as a clinical expert for nursing staff and provides targeted clinical education when needed.
- Participates in center‑based quality initiatives and promotes adherence to coding, billing, and compliance programs.
- Performs other duties as requested.
- Graduate of an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing.
- Specialization in Adult/Geriatrics and/or Family Nurse Practitioner (FNP) track; or other concentrations considered based on experience.
- Active certification as a Mental Health Nurse Practitioner.
- Prior experience in a psychiatric/mental health role.
